Proof of death

Responsibility : Le Directeur de l’état civil

In Québec, there are two official documents that can be used as proof of death:
 

  • a copy of an act of death, which is used as proof in courts of justice; 
  • a certificate of death, which confirms the date of death and is required to open the succession (estate).


To obtain either of these documents

Complete the form entitled Death: Request for Certificate or Copy of Act and submit it to the Directeur de l'État civil. You can apply online.



 

Note

Proof of death is established by the Act of Death. Only the documents issued by Directeur de l’état civil are legally recognized. It is your responsibility to find out which type of document and which format you require.
