Responsibility : Ministère de la Santé et des Services sociaux
If you wish to make an organ or tissue donation, it is important to inform your family members of your decision so that your wish is respected in the event of your death. You can choose from the following two options :
sign the consent sticker included in the brochure Please sign for life and affix it to the back of your health insurance card, at the bottom. Anyone can be considered a potential donor. However, in the case of donors under the age of 14, the sticker must be signed by the tutor;
have your notary register your consent in the Registre des consentements au don d'organes et de tissus of the Chambre des notaires du Québec when drawing up your will or mandate in case of incapacity.
When the donor dies
Before organs or tissues can be donated, a person close to the deceased must give their consent.
