Social insurance card

Responsibility : Service Canada Centre

On the death of a loved one, you must return the deceased’s social insurance card to the nearest Service Canada centre, along with a photocopy of the death certificate or the declaration of death. This reduces the risk of identity theft, and can be done in person or by mail. The mailing address is:

If you do not have the card, but know the social insurance number, write it on the photocopies of the documents. If you have neither the card nor the number, you must still notify Service Canada of the death. Upon presentation of the required documents, Service Canada can provide you with the deceased’s social insurance number, so that you can settle the succession.

Before cancelling the card, be sure to note the deceased’s social insurance number: you may need it to fulfill certain obligations related to the death.
