In Québec, two documents are required before a person's death can be officially recorded:
An attestation of death is an official document completed by a physician. It states the name and sex of the deceased, as well as the place, date and time of death;
A declaration of death is a mandatory document that is required to draw up an act of death and enter it in the Québec register of civil status.
Note
Proof of death is established by the act of death. Only documents issued by Directeur de l’état civil are legally recognized.
